Nova Scotia Health Continuing Care is pleased to share that VON, our largest nursing partner, is opening its fourth clinic location in HRM and began seeing patients on Wednesday, December 11 at 22 Glendale Avenue, Unit 3 (near the Cobequid Community Health Centre). Additional VON clinics are located in:

- Dartmouth (Dartmouth General Hospital and The Bridge)
- Membertou (200 Churchill Drive)
- Yarmouth (55 Starrs Road)
- Berwick (Western Kings Memorial Health Centre, 121 Orchard Street)
- New Minas (9319 Commercial Street)
- Pictou (91 Lawrence Boulevard, Stellarton)
Continuing Care follows a clinic-first approach to nursing services as this supports our continued efforts to deliver healthcare services to as many people in the community as possible to avoid emergency department visits and to improve patient experience. Additionally, clinic visits provide the following patient benefits:
- Flexible dates and times for appointments to suit patient needs.
- A sense of independence, empowering them to take a proactive role in their own recovery.
- An opportunity to interact with others, reducing social isolation.
- Consistent care team that will enable greater continuity of care.
- Because all medical supplies are available at the clinic, patients do not have to worry about running out of stock, needing to be at home for supply delivery or equipment pick up, having to find in-home storage space or handling unused supplies at the end of treatment.

How do I refer a patient to Continuing Care?
Nova Scotia Health prescribers can find the Continuing Care Order Form (includes referral) in the Electronic Forms Repository (EFR). Community prescribers can access this form in their EMR or through the Nova Scotia Health Medical Affairs Physician Wellness Portal. "
