Request for Care

Request for Care (RFC) is a provider-to-provider pathway established to determine follow-up care for patients who have accessed an in-person health resource and are referred by a provider for a) Chronic and Complex Health Conditions, b) Child and Maternal Health, or c) Medication and Form Requirements.  

Primary Care Clinics may receive RFC referrals from in- person health sources such as emergency departments, discharge from hospitals, cancer screening programs, or diabetes clinics. 

Starting May 21, 2025, Nova Scotia Health will be starting a phased implementation for a new, provincially standardized Request for Care form. The RFC Form will be available under "All Forms" in the EFR and accessible across all zones and the IWK. Please see thr Training Document and Provider FAQ's below.

This form is intended for healthcare providers who are requesting their patient be seen at a Primary Care Clinic (PCC) for reasons such as follow-up care for chronic and complex health conditions, child and maternal health, or medication and form requirements. 

Primary Care Clinics

A Primary Care Clinic provides person-centered, in-person, comprehensive primary health care to individuals on the Need a Family Practice Registry. An interprofessional team is resourced to deliver timely, appropriate care. Care may be provided directly by the clinic team or coordinated with other health care providers and services in the community, based on the patient’s needs.
